Parent/guardian agreement statement

As legal parent or guardian of the above child, I assume full liability of this child for the Kids League program. I further agree to submit no liability to the Hiawatha Kids League or its members which include coaches, officials, commissioner, board members and all parents of children in the Hiawatha Kids League program, including but not limited to the owners of the fields and the facilities utilized by the Hiawatha Kids League. I also agree to follow all rules and regulations of this program and that the information I provided is factual to the best of my knowledge.
